Bootleg itself claims "recorded live in Brooklyn, New York, USA."

Rather than just some random improvised solo, it's pretty clear track 2 is supposed to be "(Anesthesia) Pulling Teeth."

Track 6 fades out. "Creeping Death" abruptly cuts off at 5:10 before the song has finished completely and jumps to Kirk's guitar solo.

There was obviously an edit between tracks 4 and 5, as at the beginning of track 5 James mentions "Call of Ktulu." This is indeed an incomplete set and they played a few more songs than what's on the bootleg. I should point out that even though this was a heavily bootlegged show with several different CD and LP versions, the only additional track I've ever seen from this gig was "Phantom Lord"--almost every bootleg of this gig offers the same tracks shown above.
